We ask for as little as possible — enough to call you back, and nothing about your health. Please keep symptoms, reports and diagnoses out of the box below; they are better discussed in a consultation, where they can be asked about properly and kept in confidence.
We use what you send only to reply to your enquiry. We do not sell it, and we do not add you to a marketing list without asking. See the privacy policy for how long we keep it and how to have it deleted. Concerns about how your information has been handled go to our grievance officer at grievance@apexmultispecialtyclinic.com, who will respond within 30 days.
